Arduino Code for simple Keystroke to GPIO out

I want some simple code to run from an Arduino Uno. This must run at boot level with no OS.
I am looking to use a0-5 & 10-13 as inputs (with adjustable trigger voltage such as 5.00v on, 4.99v off) which then operates the 1-9 as outputs but in one of the configurations below;
I then want each output to be configured (easily) to do one of the following when either activated by the key or the analogue input;
1) When the input is triggered, it switches the output on and latches on, then you have to press the same key again to switch it off.
2) Hold to activate (only outputs while the matching input is triggered).
3) When a key is pressed once, I want this option to turn on then off several times (on-off-on-off-on-off....). I want the amount of times and the speed (ms) to be easily configurable with an optional longer gap between sets (eg. on/off x4 at 100ms then 1000ms gap then repeat for 3 times) this would probably need these options;
