Evaluation system

I need an o línea platform where a superuser creates groups which includes n number of questions (sentences), a logo and a list of participants where each participant has name, email and a list of evaluators (name, email and role). Whith a click, an email is sent to all evaluators with a letter and a link where they access and answer the evaluation (a scale from 1 to 5 to each question on the group). A report is generated with which evaluators have already answered and who haven't . (you can then resend the emails to those who have not answered. An excel document is generated per participant including just the grades given from all its evaluators by role.
