Existing PHP website to replace with new business model but use existing functionality

This project was started by a developer but couldn't deliver the full functionality of the requirements and someone has to pick his work and complete it. Currently there is a complete website with the required booking system already available. The problem is that this is for home massage. I need to change the existing web design and replace it with cleaners. The booking system is already available but needs to connect back to the hosting service I was using and build up again the back end based on the existing code. This means map the existing code to a PHP database that you need to rebuild it mapping it to the same code (fields). Project Duration: 1-2 weeks Project Cost: $200 ===================================================== Requirements: 1) Examine the existing code and make the appropriate links of the booking form and other pages to work live. 2) The existing php booking form connects to a mysql database via the hosting services (Justhost) however the database no longer exist. So developer needs to create a simple database tables (and the relationships, i.e. cleaners-services-location etc and then connect to the existing PHP code. 3) Amend existing code functionality and looks from a home massage website to a book cleaners. The existing functionality will remain the same however some function of the form need to be changed, for example the therapist (massage) comes for 60 – 90 – 120 mins, this to be changed so that the user can select the hours themselves, i.e. 2 hrs 3 hrs 4 hrs etc…. The rest of the functions should remain the same. 4) Change the logo to the new one on all pages. 5) Replace header and footer forms as well as all the pages and tabs with English and Polish languages including the booking button. The right wording will be provided by me in both languages. 6) Change the booking form functionality from 30-60-120 minutes to 2 hours – 3 hours – 4 hours – 5 hours – Over 5 hours 7) Change the 4th tab of the form (parts of the body) to each cleaning service. (I will provide the services, you just need to create the jpg and the functionality in the web (front end) and database (back end). 8) Change all the monetary values (money) from Euros to Pounds. 9) Fully test the complete booking form so that it records the booking in the back end admin panel. (which I don’t remember details so need to reset the access credentials) 10) Connect the existing SMS functionality (Alkatell) to the code as well as the existing Paypal account. (this already exists in the code – just needs testing that it works!) I need to be able to submit an SMS to the customer to verify his details by entering the code from Alkatell. This used to work, code should be there already. 11) There is an admin console already and there is a folder Admin (back end) and also if you go [login to view URL] the login comes up but can’t remember my password so this needs to be reset. Basically need to be able to connect in a CRM back end and add new cleaners, change availability of them, change their profiles (bio), see all bookings and create reports. So most of the work has already been delivered however I need someone who can help me quickly roll this out (1-2 weeks max) and then support can be discussed for other necessary changes. All domains, hosting service, SMS account, Paypal functionality exist and all details will be hand over to the developer. Any questions please ask.
