In need of a graphic/MS word Temple designer for a proposal cover page and headers/footers - 30/04/2018 22:01 EDT

I am currently looking for a designer to create a cover page for a document that is in word that will need to be used as a template for other documents. I also need headers and footers that I will insert into the document. I would like maybe 2 versions. Please provide examples. This is work detail:--- I will be sending you an example of what I am looking for. This project is a request for a template report that I had submitted. I would like a cover page that I am going to put into work and add the title text and detailed wording over top and I would like a header and footer that I can insert as well. I am looking for 2 renditions and I need to know an estimate of hours or fixed cost. The company is an IT healthcare based organization. The Primary colors are blue and I am including a log. Let me know what are your thoughts. And I will eventually need some white paper template work. I need the theme to be IT Healthcare but I need the capability to change out graphic and title headers as capabilities grow.
