Redesign an existing website

I am looking for someone to redesign sections of an existing ASP website. This task will be on-going for the right person. The site is under constant development, and I want my current team to work on the backend, while someone else works on the front end. Attached is a capture of the first form that I would like redone. I can supply required files and FTP access to development site so that you can test your page if need be. For your proposal to be considered, please indicate what paper is made from <-- to verify you've read the proposal. Please quote your price for this single page redesign only. We can discuss terms for ongoing development after the first task is completed. About this page: It is used for our clients to upload expense receipts. It is an overlay screen, on top of another form. What I don't like: The form looks about 20 years old. It's fully functional, but has no visual appeal. I have also uploaded a completed version of the form. I'd like to see your creative ideas and what you'd do with this form (and others like it). My ideas: Make the form more intuitive or at least insert a guide on how to complete it. The supplier field searches a database and then completes the two fields below it. If the supplier doesn't exist, the client can add the supplier. A copy of the expense must be uploaded. The reject reason is populated if the expense is rejected (client can see rejection reason).
