Web Form with Google Maps and database API Integration

The current Booking form will be replaced with the redeveloped Booking form The Booking functionality will be integrated with the Backend Booking app by implementing the provided webservices API The form to be designed to capture the 20 fields of data from the user. The main data will be: a. Itinerary information b. Multi leg itineraries - each with departure and destination, date and time. c. Dropdown list of departure and destination airports d. Google Map to display the route on the map with markers for destination and depature locations. e. Thank you page display after form submission. f. Mandatory fields, errorchecking and form validation to be implemented. Note: The CSV file will be imported for the departure and destination list of airports. Admin option to import the same will be provided. Hi, I've designed and developed a lot of similar order forms for financial companies (mostly insurance companies), so I am aware of the service design and UX aspects of the task. To start we will have to finalize the needed input fields (names, type, default, description, info text, etc) and organize them to a sales funnel and decide how to display them. If most details are decided I can create the form frontend and the api request for booking. (I'm assuming the process has to be mobile-friendly as well.) The development itself shouldn't last more than a week, but usually there is considerable time needed for planning and integrating. So a 2-4 week estimate is probably more realistic.
