Custom Wordpress Members Profile & Results Tracking Report

I have an existing wordpress membership site in the weight loss [login to view URL] site is already built, using Wishlist Member Plugin for Wordpress I need someone who has experience in adding fields to the user record, and can produce a member profile page that contains spots for member to enter weekly results. Then, calculations will be performed and displayed to the member as a spreadsheet type report. HTML, PHP, MySQL, CSS, WordPress experience needed ## Deliverables Developer will be developing on a copy of my live server. When development is done, this will become my live server. (This page will be accessible to the user/member ONLY) Member Name (prefilled) Member Picture (ability to upload) Date Started (ability to enter this) My Why (text field that can take up to 500 chars) My Commitment (see attached) Results tracking (see attached spreadsheet). Here the member will have the ability to enter weekly info, similar to what is on the attached [login to view URL] info will produce Spreadsheet type calculations that track the members progress, and presents the results of the calculations back to the user. Need the ability to track the info for 52 weeks. User would be able to scroll through the info, keeping the beginning column fixed. NOTE: The beginning column will be pre-filled with the Date Started they entered above
