Upload images using PHP with progress bar using ajax and/or jquery

We require someone to modify/write an existing PHP / jQuery/ajax script/Library to upload ACCOUNTS PAYABLE-payments AND Accounts Receivable payments from o our Mysql and Minors fixes to the form to save payment from vendors and customers We would like the layout to permit :BROWSE,UPLOAD, DOWNLOAD, PREVIEW, DELETE - The user browse for files by selecting Browse. - When the user selects Browse, before the mage is uploaded a preview of the new thumal of the image is displayed - The image uploaded when the user selects the 'Upload Image' button. - We would like a small progress bar under each file while it is being uploaded (Similar to Facebook). - After the user uploads the image is uploaded a preview of the uploaded image is shown - The uploaded pictures files must be stored on our Mysql - The code must be 100% error free (Not suppressed from not showing errors) and must be Integrated to work with our existing form.
