AJAX Document Uploader Form

I need a popup form that will allow the user to upload documents into a folder, and enter a record in the database of the file path. - User clicks ADD DOCUMENT button. - Popup prompts them to add file, select the type of document, and enter any relevant notes - When the user clicks 'Upload', AJAX is used to upload the file into the directory, and to update the MySQL table with the information. - A visual representation of a progress bar should be displayed. - Once complete, the new record should appear under the existing records instantly - without the page reloading. - Each item will contain 2 buttons - one to download the file, and the other to delete the file/table item. See attached image. This solution must be capable of uploading files that are several megabytes in size. I have had someone else attempt a solution but when you upload files that are more than a couple megabytes, the solution does not work. The MySQL table already exists. The PHP solution which pulls existing documents from the database is in place. This is to add to the existing code.
