Build Dynamic Logic Form

Desired Function Ability to dynamic add/remove form fields Ability to add/remove groups Ability to change dropdown and field types based on selected dropdown condition As user, I’d like to select various conditions from a drop down. Depending on the dropdown type (A in picture) (number, text, date) the conditionals (B in picture ) change and the input field C changes to that type. (see [login to view URL] for reference) When clicking “Add Condition” a new set of fields is added that I can make more selections. If I press the Add Group, my previously selected conditions are grouped together and a new group is created and I’m given the option to select the Conditionals to tie the groups together. This form will be posted to a PHP file. For this example a simple print_r($_POST); page is fine
