Change control website

We want to produce a website 'Health Check' process that allow us to check our clients WordPress websites using a specific checklist which is then followed by a staff member and they confirm each step in the Health Check process and make notes if they find anything of concern which the client should be aware of. At the end of the Health Check process a PDF report is emailed to the client confirming all the website checks that were made and any notes regardig changes that were made to the WordPress plugins, scripts and security on their website. The project will require creating a MySQL database that covers all the Health Check process, clients contact details and User access. The website front end will cover: login page Select or Add a website for the Health Check. A Health Check page showing up to 10 different checks to be covered with confirmaton buttons for each check and note field. As each button is checked on a section this is saved to the datebase and date and time stamped. A final submit button which will send the Health Check report to the client at the end. Produce the PDF report file and email. Be able to list and access previous Health Checks for each website
