I have a contact form the files for which are attached. Once the form has been completed and sent it is held in a temporary file until the sender has verified their email address and then it is sent to the recipient. The form and therefore the details it contains needs to be increased with the addition of the following fields: Address Address2 City Postcode Telephone and a drop down menu with 4 options. The drop down menu has a title: 'Please tell us how you would like us to contact you' 1. Please choose (default) 2. By Email 3. By Telephone 4. By Post 5. Do not contact me. The drop down menu and all fields (except Address2) are required (mandatory) I also require the code to be changed for Re-Captcha. There is a full description of the work which you MUST read before responding. If you have any questions you MUST ask them before bidding. You MUST have full command of the English language and demonstrate same in your resepnse. Do NOT send me an advertisement, send me a quote which reflects your understanding of the work required.

I have read your requirements in detail i.e. When a user submits the form its held in a temp file and not sent to recipient until he/she verifies their email. Once verified the associated form held in temp file is then sent to the user email. We need to add 6 more fields to the form and same fields will be added to the email that is sent to the user and also added to the temp file. The form that you have now, when you fill it, the data is sended by email or saved in a database? The recaptcha that you have now it's a version 1 or 2?
