php based web application

The project is USER -------------> CREATES PATIENT (REGISTRATION)---------------------> ADD ASSESSMENT , CHOOSE DEPARTMENT, CHOOSE TYPE OF ASSESSMENT. These assessment should be easily searchable The assessemt added would choose its date and time automatically but can be modified user can create as many assessment as he wants. The assessment would be about 50-60 forms based on java and needs to be having database. We should be able to export the form to excel We should be able to export assessment of same type of same patient into one excel We should be able to export assessment of same disease patient we should be able to export assessment of multiple patient of same type of assessment we should be able to export assessment of same location patient and their same type of assessment we will provide you assessment in infopath forms
