I have good experience with DICOM and Medical Imaging as I worked with GE and Philips healthcare in the past for about 15 years on their imaging products. Recently also I was working with Open source DICOM technologies, helping my clients build imaging solutions. I went thru your specs document. One thing that is not clear is where are the actual DICOM files stored. Are they stored in cloud storage like S3 ? I understand that the postgres db is going to have only the patient, study, series and instance attributes, but to serve the WADO requests, you need the full DICOM images to be stored at some place. Another question I have is whether your WADO requests are authenticated ? I assume so, if not you can't support multi tenancy. What kind of authentication mechanism you use ? Some kind of header or cookie ? How does the DICOM image gets to the server ? Is there an upload process with the full image and the details get parsed on the server side and the entries in the db gets created ? Or the study/series/images entries are created by directly calling the service directly ? You are also talking about GDPR and de-identifying the images. You are only looking at DICOM header level deidentification or also looking at burnt pixels which might have PHI ? I understand your scaling needs.. We can create docker containers for the service which can be deployed on a swarm or kubernetes cluster, based on our scaling needs. Lets discuss in detail.
