Simple database design with address label template

I need a simple template creating to allow me to print of address labels. I will supply the template that I need you to place on the [url removed, login to view] linked to the address label are Title (Director/Co Sec), Company, Address, Town and Postcode.

This is a simple 30 minute job for the right person.

Please do not bid above $30 as your bid will be rejected.

The right person will get another larger job from me within 1 month.

Please specify what program you will be using for the database and why?. I use Filemaker and Access but will use another program if required.
