PHP Web Scraping 3 (script creation)

I need a PHP script created to scrape these pages for event data:

The script should parse through events from each state via this URL:

Make sure to grab URL for the event/organization logo.
The data will need to be pushed to a staging table initially (you'll need to create it), then to another table, structure below:

The script needs to pull all pages.
The fields you'll need to populate from this script are:
Title
Description
StartDate
EndDate
StartTime
EndTime
MinAge
MaxAge
Cost
Address fields
URL (this should point to the individual event)
OrganizationName (this will be from each URL that is searched in the URL table)
Image (Link to the Logo)
Final delivery should include:
Table structures in SQL for source data and URL table.
Scraping Script
ETL Script
Spreadsheet of Events for final review.
I will ignore anything without a timeline. My specs should be very clear, if not, please ask [url removed, login to view] project uses PHP.

I have a lot of experience in writing parsers, crawlers and scraping scripts. What is ETL script? Can it all be done in one single script?
I will create the log system that will insert the event data in the database table events_copy. I will be able to deliver today before midnight (GMT).
