IT4IT Standard Requirements Extracted into Excel Format

We are the IT organisation of an Australian financial services company and are looking to improve our service delivery using The Open Group's IT4IT Reference Architecture.
The IT4IT Reference Architecture is a standard for how an IT organisation should operate. The standard itself and it's requirements are in a PDF (attached) & HTML format (available here: [login to view URL]).
We need the core set of requirements found in the document to be extracted and converted to an Excel format that will allow us to evaluate how well we comply with each requirement for a given area. E.g. Do we comply with each clause of the standard in the area of Incident Management? Which team is involved etc?
There are 33 sections of the standard that we need extracted. Specifically sections:
5.4.1 to 5.4.6
6.4.1 to 6.4.9
7.4.1 to 7.4.9
8.4.1 to 8.4.9
Each section has three "elements" and each element has a series of "requirements" or "criteria". Specifically, each section has the following elements:
- Key Data Objects List
- Key Data Object Relationships
- Functional Criteria List
- Data Architecture Criteria
We would like a single Excel workbook, with each sheet covering one section and the list of items under each element as rows in the sheet in Column 2 with the parent element noted in Column 1 for ease of filtering. A sample has been provided with one section completed.
We would also like a "Summary" sheet which list all the criteria linked by formulas to each sub-sheet.
