Project 122 - PDF Creation expert

I have a pdf created of 8 business cards on a page. I need to add a few text fields to the pdf. When a user fills out the text field on the pdf, it will apply to all 8 business card files.

If you can do this ASAP... basically right now, bid.

Also, after the pdf text fields are filled out, we would like to lock the fields so no one can change it.
