Build an online questionnaire form like the following link:

All questions will be required. If not, a message will appear for each missing unanswered question.
The form will email the answers to an email address. (This function will be provided).
Submit screen splash will display as the one shown in the link.
Will supply the submitted page.
All design will be done by bus and sent to you.
Programmed in ASP.NET
All you need to program is the form submit check and indicate which questions were not submitted.
All work will be required to work on our servers.
