Design a Website Mockup

i need a 4 page website building it is a online application form. on the home page will be a job discription that i have the content for alon with some vewry nice design work to make it eye catching please look at this site i saw on the internet [login to view URL] this has been done by a proffesinol graphic designer and this is how i want mine to look obviously not the exact same but just as good on the 2nd page i need a FAQ page for phone sexi have all the FAQ content on the 3rd page i need another FAQ page for webcams i have all the FAQ content on the 4th page it will have to be a application form that when users fills in it sends to my email address on application form is very simple ie name.... email.... age.... job (phone sex) or ( webcam ) <<<< tick box skype ID.... location..... were did you hear about us........ add a short discription about your self [ ] [submit tab] need this job doing in 24 hours its really urgent
