I have PHP Wish Website I Want to Modify Add a Shortcut Button on Dashboard

I have a wish Website called [login to view URL] I used 2 type of wish here single name wish and double name wish single wish the only username used here like [login to view URL] Check Its Only 1 Name Show using here only 1 Input which has an option to make a new wish in Website Dashboard See Image [login to view URL] here is the issue Double Name Menu options not added on Dashboard double Name Example - [login to view URL] here is 2 input 1st Friend name & 2nd is Your name(Username) so i have issue on Duplicate and change code when making 2nd Input Wish so I want to Someone add an option to make quickly Double Name Wish In Dashboard, So when I need to create one then quicky create through button which are in dashbaord all are works in single template call [login to view URL] so iam not able to fix myself and its May Be Need to Add Any External code so if anyone will do i will be appreciate and I give u Good Review & Rating 5 Star
