Help to correct some code errors

I have modal that holds three icons buttons, users are supposed to select any of their choice and when the icon is selected by the user it will be inserted into the database then the selected icon will appear on datalist page including the name. I have issues that came up on the insert, the two icons insert well and display on page, the book and the cup, but the third icon which is pen is not inserting. Secondly, i want same icons on modal not just to be inserting but to act as update button, that means when i click book icon on modal it inserts book into table then displays it on modal, when i click same book icon or any icon on same modal it updates the record. 3. Each inserted icon must appear on datalist with its color,(Just modify my code to make the selected icons on datalist to appear with its color) 4. Hid image and video controls that are not in database, instead of seeing empty image and video control on post hided them, if video or image in table doesn't exist hid. 5. I have a problem on signup form, when a user signs up confirmation email is sent to the user so that when the user clicks on the link the user account will be activated. I have done the form , where am having issues is the code for sending email to the user, its showing some error.
