Laraval Squid Proxy panel - MYSQL - db_auth problem

hi,
I have two Laravel-5 panels ( Client and Admin panel ) with all codes on cpanel saved.
MYSQL user and db with the port is not connecting on Linux Proxy server.
This is error : Access denied for user 'username'@'10.3.x.x' (using password: YES)
2nd issue is : FATAL: Authentication helper program /usr/lib64/squid/squid_db
