i require a single html or php page to be used as a form for input of information from clients. the page will have be as follows [login to view URL] division will be 960px wide 2. the form will be divided into have 3 sections 3. it will have string/text input fields, it will have a number input field 4. it will have a date field + date picker 5. it will have check box 6. all fields will be required therefore it will be validated 7. it have 4 pdf and .doc attachments input fields 8. on clickin submit form will send all information to an email in a formatted and presentable way and show a success result on screen
