Registration Web Page

Environment: The site uses BootStrap’s HTML, CSS, and JS framework. The Web server is Microsoft Server 2012 ASP.NET 4.0 and up. Database is SQL Server 2012. User Registration User will be able to enter in the following information: • First Name • Last Name • Address 1 • Address 2 • State (Drop-down data comes from database) • City (Drop-down data comes from database when state is selected) • Zip code • Phone (Home) • Phone (Cell) • Email address • Occupation (Drop-down data comes from database) • Gender • Password (Password will check to make sure it is a strong password and that it meets requirement) • Password confirmation • Terms agreed check box Once the user clicks on the submit button, it will check to see if the email address already exist. With it does, they will be redirected to the page with a message on top stating an email already exist. Click here to login or click here if you forgot your password. • Log in screen – Allows the user to log in with an existing email and password. • Forgot password – Allows the user to enter in the email address and their Epi-ID and an email will be sent to them requesting they click on the link and requesting they enter in a new password and confirm the new password and submit it. It will check to see if there’s an existing password in the database. An email will be sent to them that their email was changed. • Forgot Epi-ID – If the user forgot their Epi-ID, the can have it emailed to them on a separate page by entering their Email address o Epi-ID – System generated ID once the user clicks on the conformational email link.  Example: EPIU0320DX4 If the user information is submitted ok, they will be redirected to a Web page. An email will be sent that they have registered and to click on the link to check if they are they person they say they are. Once they are directed to the Web page, it will update the database stating they have confirmed who they are. The system will generate an Epi-ID and display it on the Web page. Doctor Registration Doctor will be able to enter in the following information: • First Name • Last Name • Hospital/Practice Name • Address 1 • Address 2 • State (Drop-down data comes from database) • City (Drop-down data comes from database when state is selected) • Zip code • Phone (Home) • Phone (Office) • Phone (Cell) • Email address • Practice (Drop-down data comes from database) • Gender • Password (Password will check to make sure it is a strong password and that it meets requirement) • Password confirmation • Terms agreed check box Once the Doctor clicks on the submit button, it will check to see if the email address already exist.
