Short term memory survey app for phone.

My name is Zachary and I am looking for a Freelancer to develop an app for my ill wife which is part survey, and part memory test. I want too create some sort of form. that every 15 minutes will ask her questions like "How tired do you feel? 1 is worst, 10 is best" "have you fainted since the last time you filled this form?" "in one sentence or so, explain best how you feel" "What was the last memory test words?" "here is your new memory test words //Red banana// (as an example). I want 2 random words to be generated. ideally simple words. Each time she opens the app. I want the form to generate 2 random words. I want the random words to stay saved on that specific form that she fills out. I would like each form to be saved/sent directly to me. My wife's health is critically poor and the US health system is ass. We have appointments over the next few weeks on tests they are going to do. Since march she has been having these 2 day episodes, where during this 2 day period she faints between 1-10 times a day with completely memory loss up to 30 minutes before and after fainting. and even inbetween fainting she will forget if she ate, etc. I have noticed a correlation between how tired/exhausted she appears and how soon she will faint again. So I am trying to get proof of this correlation to give to doctors, and also trying to see if we can predict when she will faint and have memory loss. These 2 day episodes started off exactly 7 days apart. 7 7 6 6 5 5 4 3 4 3 2 2 2 1 1 2 1 brings us to current day. Aka this week. Monday Tuesday she was in an episode, passed out 1 time monday, 7 times Tuesday. Wednesday she felt fine. today (thursday) she has passed out 3 times so far. Moreover, the platform that I need this app to be built on should be Android. The attached files is a basic outline of what I want there. however the questions are exactly what I want unless I agree to something else, however I may have typos, but I do not want you to just create your own test, there is specific reasons each question is on there. I do not care about if it looks pretty/good. I care about functionality that the form can be filled out, each time it gets filled out the form is saved and/or sent to me. And that each time the form is filled out, it has 2 different words generated that saves on the submitted form, otherwise I have no way to track is she got the correct answer or not. It asks me if i think this is a micro project or small project when monetary ranges between each. As i can do a google survey in 10 minutes that gives me the first 75% of what im looking for (yes i know the hardest part is the 25% I can't do) I am not sure how much of a project this is, however I would expect it to be quite easy for someone who knows how to program. I could be wrong. My understanding is this would not take very long at all.
