PHP Forms, Resume Search Site(repost)

I want to add the following page to my website: Our company specializes in both Information Technology Services and Validation Services. Professionals in the area of Information Technology or Validation Services will occasionally browse our site, and we want pages were they can submit their resume. I am attaching two forms, one is an IT resume submition form and one is a Validation Services form. Your job is to convert these forms to php forms, collect the information, and store them in a mySQL database. I am looking for a creative programmer that will design the php form in a user friendly fashion, as well as make the pages look good graphically. After this is done, I will need the following: Since we outsource human resources,I want our customers to be able to go into the site and search our database for different skills that they might be looking for. You will make a web application that will allow our customers to search through the database for a particular skill. You will not reveal any of the candidates personal or contact information. Only after they contact us will they be given the candidates contact info. Again, I am looking for a user friendly design that will allow the customer to quickly look through the DB. Once they get a match for those skills, a list of the qualified candidates will appear, and they can click on that cadidate to look at more details regarding their qualifications. The site is on a linux server with PHP and MySQL installed,no ASP. ## Deliverables Two php pages that will be linked to my site. The form that clients will use to search through our DB for particular skills. 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform All Major Internet Browsers
