Knowlege base- html/php form that allow query and to post on SQL

I currently own a web host and need someone to make me a webpage that would allow visitors to search information from a database and also input/edit information. I am planning to make a simple knowledge base. I want it simple and straight to the point. -All will have 3 parts, title, tags and body. -There will always be a search bar on the top of the page which will contain a search box, 3 check boxes(TITLE, TAGS and BODY) and 2 button links(SHOW ALL and ADD ENTRY) -The search bar will only search which ever check box is check... For example: if only the "TITLE is checked, the search will only query titles. -The query will yield a result with the highest amout of matched words on top. -The results will show 2 parts, title on the left side and part of the body on the right. -The results are going to be a link to the full information. -The information page will have a button at the bottom that will allow visitors to edit the info
