Fancy Splash Page and HTML Programming

I need a single splash page built with some unique functionality. I have general samples of what I want it to look like, however will hold you to add your own signature of creative influence. The page is unique in that I need some functionality with respect to an inset data form that will ask the website visitor some general questions surrounding the topic of tax. Depending on the answers, I will need a small variation of follow up functions depending on what and how the questions are answered. I know very little about HTML programming and web design, so I would liken my need to that of an IF/THEN set of programmable scenarios. If the project goes well, then there could be more back end work in connection to an Iphone app that we want to integrate in a similar fashion. That said, my time frame is short, like by end of day tomorrow. I previously experienced on this site an under promise of delivery, which in the end created tension and serious disappointment when this person failed to meet the deadline. If this timeline is not realistic for you, DO NOT BID on this project as I will completely hold you accountable.
