Website to Capture User Data

My proposed website is aimed at capturing tax payers' information with the following features: a) The tax payers will buy a scratch code with a PIN, then logon to the website. If they are not yet registered, they will click the register now link which will ask them for a valid PIN. If the PIN is okay and has not been used for registration, then the user will be taken to a registration page where they register and supply all necessary data required. During the registration process, the user will choose a username and password and a valid email address. If registration is successful, the PIN number is invalidated and can no longer used for new registration. Next time the user logs in, he only needs to enter the username and password chosen during registration. b) Tax Payer that have successfully registered will be issued a unique ID called Tax ID. The information will be displayed on the screen after registration is successful (eg You are successfully registered. Your Tax ID is ??????. Please print this page or write down this ID for easy reference). Also copy of the Tax ID will be emailed to the user along with the other registration information such as username and password c) The home page of the website should prompt visitors to choose a STATE from a list. So they must be registered under one of the listed states. Also I need that the look of each State will be different with the ability to load the logo for each state. So when a visitor logs into a State, he sees the logo of the state. Nevertheless, the Tax ID generated will be unique across all the States ie there will be no two IDs that will be the same even though there are several States d)Once the user chooses a State and Logs in, there will be menu option for Corporate Users, then Individuals. Access to these links will be for only logged in users. ## Deliverables For the CORPORATE USERS, the following information will be supplied ? - Name of Company (this will appear in a list when individuals are registering below) ? - Type of Business (from a list setup at the backend) ? - Primary Address of Business ? - No of Offices ? - Staff Strenght ? - Average Gross Monthly Payroll ? - Contact Person (Full Names) ? - Designation ? - Telephone No ? - Alternate Telephone No ? - Email ? - Alternate Email FOR THE INDIVIDUAL USERS, the following will need to be supplied ? - Name of Company (from the list of companies that have registered ? - Full Name ? - Nationality ? - Resident Permit (non-Nigerians): grey this field out if Nationality chosen is Nigerian ? - Home Address (Not PO Box) ? - Tel Number ? - Alternate Tel Number ? - Email ? - Alternate Email ? - Job Position (choose from List) ? e) I expect the Backend to have provision to ? - Create/Edit Business Type ? - Create / Edit Job Position ? - Manage PINs (genertae PIN, Activate PIN) The PIN list should show Batch No, Serial Nos, PIN number and status of the PIN whether Inactive, Active or Void. Void pins are those that have already been used to register ? - Newsletter: So that we can send newsletter to users from time to time ? - Upload logos for States f) Backend ? to show no of Individual Registered Users and no of Corporate registered users. g) Report at the bank end to show details of registered users: Tax ID, Reg Type (Individual or Corporate), Contact Person, Address, Tel No, Email, Type of Business (if Corporate) h) Export report in various format (txt, excel, ascii, pdf etc)
