Searchable Database with Points System - repost

Hi there, I need to have a Database and forms built for a People Search system. Basically, I want to offer information I have about people at a cost of 50 points a search. It will be sort of like a skip tracing system used by bill collectors, car lots and such. This will be integrated into a Wordpress site and will use the current theme. Customers will be added to a membership system, where they will log-in using their account number, name and password. Upon successful login, they will be told of how many points they have and will be directed to a database search form. The form will have the search fields: First Name, Middle Name, Last Name, Street, City, Zip, SSN...none of which are mandatory...there will be a checkbox to check the system using the SSN only. That will grey-out the other boxes. Customers will enter search information: this search information will be captured and added to the database as well...the captured information can go into a separate database to be added with a batch job every 24 hours, or however it can be done. The search will return with whatever data results are available. There should be an option to print those results 50 points should be deducted for each search and the member should always be made aware of how many points they have left. When there are no more points left, the member should need to log-out of the system. There should be an admin form that allows for database to be added, edited and deleted. This form will have the same fields as the search form, but will also include a place to add 3 aliases names and addresses. I have attached some mock-ups I have done to give an idea of what I am looking for. I am looking to have this done in 1 day. Thanks!
