PHP/MySQL Signup Script with Code Validation.

I require a Registration form with following Fields: LOGIN DETAILS ___________________________________________ Username: Password: PERSONAL DETAILS ___________________________________________ Full Name: Email Address: Mobile Number: Address: City State Country: Postal/Zip Code: _____________________________________________ PREMIUM ACCESS Invitation Code: (10 Digit Alpha-Numeric Code) A code generator, which will generate 10 digit code and store it and can be seen by admin anytime and can be downloaded as well. When these codes are generated and used in the registration form in "Invitation code", the registration is fully activated. If they do not enter Invitation Code, they still get registered, but will not be able to see selected pages of the website. When they click the menu for restricted pages, it should ask for Invitation code. Once they enter the code, it is validated with the generated codes and get full access to all the pages of the website. Once the code is used and activated, it cannot be used again. So 1 code per user. Fixed Budget, no Bargain. Payment will be released only after successful testing and satisfaction.
