Write a simple PHP script (to be completed within 24 hours!)

We have the whole thing working in MS excel (attached here) and the accompanying 7-minute video explains how it works. As you can see it's actually a very simple system. We just need you to write this as a PHP script and then test everything properly to make sure everything works as it's supposed to. This will be a very simple script and if you know what you're doing, it shouldn't take you longer than a few hours to complete and test everything.
