Login form in VB.NET

Hey guys, I need to get a Login Form for my app in vb.net. I need it to access a Access Database that will have two coloumns, Username/Password. The user will type in there username and password and hit login. If it is in the database it will load the next form, if not prompt them and say incorrect Username/Password. I will also need a form that I will be able to add the usernames/password into the access file with. It will be a seperate file as it will be part of the master program. So what i need is a Login form that I can add to my existing project and a totally seperate form that I can add users with. Need this ASAP
