Rewrite (where needed) tiny PHP and TSQL based page in proper PHP

I have a small PHP page which pulls date from a TSQL database, formats it and presents it in a layout I've designed for printing or saving. The PHP is very poor, and the entire thing is amateur, but it's always worked. It uses sqlsrv_connect() and it fails when I attempt to use the same PHP on a modern server with IIS and PHP5+ I have rewritten the logic that makes the PHP complicated into a Stored Procedure (This work is already done) so all I need is: Script does some basic input checking, returning a form for entry of variables if it fails. If validated, it performs the query and populates a table with the results. The layout is fairly simple (very few CSS elements are used) and I'm happy playing with that after the results are at least being returned correctly. I can provide the current PHP, and screenshots of the final layout if requested. I anticipate this being nice and simple for someone with the right skills but I am not that person. I cannot give a copy of the database being used, but I can give the output from the SP which is consistent.
